Data Timeline

Please  select a date in the applicable year between July 1 and August 31 that represents the end of a payroll period. For example, if you have a payroll period that ends on August 15, 2025, it would then go back one year to August 16, 2024. This is the year that would represent the reporting period for the 2025 filing cycle. In addition, you can use December 31 of the previous year. 


You do not need to use the same date that was used in the previous filing year.

Workforce, Hires, and Terminations Sheets

Employee ID

A Unique identifier for each employee. This data is not included in the final VETS 4212 report, so if you would like to "anonymize" the Employee IDs, please do so as long as you can still identify each employee when checking your data.

Establishment Name

An Establishment Name should be listed for each employee, and is the physical worksite the employee works at. If the employee is remote, please assign them to the physical location they would work at if they were not a remote employee.

EEO-1 Category

Every employee should be assigned to an EEO-1 category. The EEO-1 categories and template codes are as follows (please do not include the quotation marks in the template.):


 Executive/Senior Level Officials and Managers : Coded as "OM1" in the data template.

 First/Mid-Level Officials and Managers : Coded as "OM2" in the data template.

 Professionals : Coded as "PROF" in the data template.

 Technicians: Coded as "TECH" in the data template.

 Sales Workers : Coded as "SALES" in the data template.

 Administrative Support Workers : Coded as "ADMIN" in the data template.

 Craft Workers : Coded as "CW" in the data template.

 Operatives : Coded as "OPER" in the data template.

 Laborers/Helpers : Coded as "LABOR" in the data template.

 Service Workers : Coded as "SERV" in the data template.


Veteran? (Y/N)

Please list "Y" if the employee is a veteran, or "N" if the employee is not a veteran.


Hires

Please list all employees that were hired in the 12 month period preceding the date chosen as the payroll period end date. For example, if August 15, 2025 was chosen as the payroll period end date (or snapshot date,) please list all hires that occurred from August 16, 2024 to August 15, 2025. (reporting period.)


Hire Date

Please list the date each hire was included in the payroll for the first time during the reporting period. 


Hire Date should be in MM/DD/YYYY format.

Terminations

Please list all employees that were terminated in the 12 month period preceding the date chosen as the payroll period end date. For example, if August 15, 2025 was chosen as the payroll period end date (or snapshot date,) please list all terminations that occurred from August 16, 2024 to August 15, 2025. (reporting period.)


Termination Date

Please list the date each termination occurred.

Locations

The Establishment Names of each location will auto-populate based on the establishments listed in the Workforce sheet.

Address Information

Please provide the address information  for all Locations and Parent Company Location.


Please ensure the only special characters in the street address are  apostrophe ('), dash (-), period (.) 


Address Line 2 is not required.

Company Contact

 For purposes of completing the VETS-4212 Report, enter the name, telephone number, and email address for the "Company Contact" for the employer for which the VETS-4212 Report is filed. The "Company Contact" is the primary point of contact who is responsible for the management of the company, report, and user information which is submitted through the VETS-4212 Reporting Application. The "Company Contact" can be removed or designated by the registered user only. Users will need to contact the VETS-4212 Service Desk and register with the application in order to remove or change a "Company Contact." 


The telephone number should be ten digits. For example 5555555555.

HQ Location? (Y/N)

Please list "Y" if the establishment is the company headquarters. Otherwise please list "N" for no.

Prime Contractor/Sub-Contractor? (P/S/B)

Please list "P" for Prime Contractor, "S" for Subcontractor, or "B" for both.


 A prime contractor is the company which was initially awarded a contract to provide a service or product to the federal government. The Vietnam Era Veterans’ Readjustment Act (VEVRAA) requires covered federal contractors and subcontractors to file the VETS-4212 Report. The VETS-4212 Report requires a company to indicate the type of contractual relationship that it has with the federal government.


 A subcontractor is a business who receives a contract from a federal contractor (prime contractor or subcontractor) to perform a specific task as part of a contract awarded by the federal government. The Vietnam Era Veterans’ Readjustment Act (VEVRAA) requires covered federal contractors and subcontractors to file the VETS-4212 Report. The VETS-4212 Report requires a company to indicate the type of contractual relationship that it has with the federal government. 

NAICS Code

Please list "P" for Prime Contractor, "S" for Subcontractor, or "B" for both.


 The North American Industry Classification System (NAICS) is the standard used by federal statistical agencies in classifying business establishments for the purpose of collecting, analyzing, and publishing statistical data related to the U.S. business economy. The NAICS code number used should be the six digit number applicable to the Establishment Name for which the report is filed. If the you cannot find a separate NAICS code number for the Establishment Name, enter the NAICS code number for the parent company. Learn more about NAICS codes on the U.S. Census Bureau's website.  

EIN

 Employers must include an Employer Identification Number (EIN) on the VETS-4212 Report.  The EIN should be a nine digit number applicable to the Establishment Name for which the report is filed. If there is not a separate EIN number for the Establishment Name, please enter the EIN for the parent company.  Learn how to obtain an EIN from the Internal Revenue Service (IRS). 

DUNS

 Employers must include the Dun and Bradstreet (D&B) identification (DUNS) number on the VETS-4212 Report. The DUNS number should be the nine digit number applicable to the Establishment Name for which the report is filed. Companies will be required to obtain a new DUNS number if there is not a separate DUNS number for the Establishment Name. A DUNS Number can be requested at no cost online on the D&B Company website.  If you cannot obtain a new DUNS number for the Establishment Name, enter the DUNS number for the parent company.  

Snapshot End Date

Please  select a date in the applicable year between July 1 and August 31 that represents the end of a payroll period.

Company Number

 If a VETS-4212 Report has been filed in the past, you will need to enter the company number assigned in previously submitted reports. The company number is a company identifier used when filing the VETS-4212 report. The company number should start with a letter "T" followed by six digits. A company that filed in 2024 should use the same company number when filing in 2025. If a company number is not available, please leave the field blank. If there are any questions regarding a company number, please contact the VETS-4212 Customer Support Center at 1-866-237-0275 or vets4212-customersupport@dolncc.dol.gov.
